How can I always open a new tab when I click [command + Class]? Sometimes it opens in a new tab, but other times it doesn’t, as shown in the attached video.

It's because the tab is in "preview" mode (the name is in italic).
You have to double-click on the tab to make it "permanent"

You can also disable preview tab functionality in the first place, but it'll also e.g. not show you a preview when you select a file in project tree afair
