I have to say.. The Codex integration on ktor’s PRs is _really_ good. 

<@U0653A2V2QK> So apparently I'm just determined to break concurrent startup mode. After you merged KTOR-9889, I found another issue with concurrent loading. This time if you have chained dependencies:
```application {
  dependencies.resolve<BankService>()
}
application {
  dependencies.resolve<GreetingService>()
  dependencies.provide<BankService> { BankServiceImpl() }
}
application {
  dependencies.provide<GreetingService>()
}```
This one will fail with a MissingDependencyException due to `BankService` being missing even though the second block provides it. I've found _why_ it does it, but fixing it is the hard part. Any thoughts?

oh, interesting... it looks like for supporting this scenario it will need multiple rounds of suspension before it gives up on the placeholders

Pushed a PR that attempts to solve this, but there’s a lot of issues with it. It might need some more discussion.
